just like nursing notes in the hospital! what happened, how the patient reacted, what did you do ..

the first time i was told that my writing was good was from a nurses note, that saved the hospital $$ because i wrote exactly the facts and when the patient tried to cause problems they used my note in court.. i was shocked.

now i go all over and sometimes in order. but this story was a great reminder for something i am working on, get the facts, the order the methodology then add fluff *if needed.
